This form should only be used by facilitators/monitor for requesting the withdrawal of students from Georgia Credit Recovery courses. Requests from students or parents will not be honored.
Technical issues including enrollment of students, viewing of content, or general credit recovery questions should be addressed by filling out a inquiry for a specific class at Inquiries and Test Resets.
